Взаимосвязь приверженности лечению и качества жизни пациентов с артериальной гипертензией

Аннотация

Артериальная гипертензия (АГ) остается ведущей причиной глобальной смертности и инвалидности, поражая более 1,3 млрд взрослых во всем мире. Несмотря на доказанную эффективность антигипертензивной фармакотерапии, целевого контроля артериального давления (АД) достигают лишь около 20–21 % пациентов, что во многом обусловлено низкой приверженностью лечению. В настоящем обзоре анализируется двунаправленная взаимосвязь между приверженностью терапии и качеством жизни (КЖ) пациентов с АГ. На основании данных из различных регионов мира показано, что высокая приверженность не только обеспечивает стабильный контроль АД, снижение частоты инсульта, инфаркта миокарда и прогрессирования хронической болезни почек, но и значимо улучшает физическое, психологическое и социальное благополучие пациентов. Приверженные пациенты реже предъявляют жалобы, демонстрируют более низкий уровень тревоги и депрессии, а также ощущают контроль над хроническим заболеванием. Напротив, низкая приверженность формирует порочный круг с ухудшением КЖ, повышением риска осложнений и ростом затрат на здравоохранение. Кроме того подробно рассмотрены основные барьеры приверженности, представлены современные стратегии ее повышения: использование фиксированных комбинаций антигипертензивных препаратов; цифровые технологии; применение инструментов искусственного интеллекта; химический мониторинг; образовательные и психосоциальные вмешательства. В целом, повышение приверженности лечению при АГ является не только способом улучшения контроля АД, но и мощным инструментом повышения КЖ пациентов, требующим перехода от модели «комплаенса» к партнерской модели «конкордантности».
