Прогностическая роль фактора некроза опухоли альфа у больных со стабильной ишемической болезнью сердца и различными вариантами поражения коронарных артерий

Аннотация

Цель     Оценка уровня фактора некроза опухоли альфа (ФНО-α) и его прогностической роли у больных со стабильной ишемической болезнью сердца (ИБС) и различными вариантами поражения коронарных артерий (КА): обструктивным и необструктивным.

Материал и методы     В обсервационное аналитическое когортное проспективное сравнительное исследование были включены 90 пациентов в возрасте 40–75 лет со стабильной ИБС. Пациенты были разделены на 2 группы в зависимости от степени обструкции КА по данным коронарографии или мультиспиральной компьютерной томографии КА: пациенты с необструктивным (ишемия и необструктивное поражение коронарных артерий – ​ИНОКА; n=40) и обструктивным (ишемическая болезнь сердца с обструктивным поражением коронарных артерий – оИБС; n=50) поражением КА. Всем пациентам проводилась оценка уровней ФНО-α и фактора роста эндотелия сосудов (VEGF) методом иммуноферментного анализа. Период наблюдения составил 24 мес. В качестве первичных конечных точек у всех пациентов оценивались летальность, частота экстренных госпитализаций по поводу инфаркта миокарда (ИМ) или нестабильной стенокардии, необходимость проведения плановой коронарной реваскуляризации, частота развития хронической сердечной недостаточности.

Результаты   В группе ИНОКА преобладали женщины (n=23; 57,5%), тогда как в группе больных с оИБС – мужчины (n=35; 70%; p=0,009). У пациентов в группе ИНОКА отмечался более высокий уровень общего холестерина – ​ХС (p<0,001), ХС липопротеидов низкой плотности – ​ЛНП (p=0,001) и ХС липопротеидов высокой плотности – ​ЛВП (p=0,002). Уровень ФНО-α в группе пациентов с ИНОКА был выше и составил 28,17 [14,35; 29,79] пг/мл, а в группе оИБС – 12,95 [10,56; 22,80] пг/мл (p<0,001). При корреляционном анализе в группе ИНОКА отмечены умеренная прямая связь уровней ФНО-α и VEGF (ρ=0,332; p=0,045), слабые связи с уровнем общего ХС (ρ=0,284, p=0,094), ХС ЛНП (ρ=0,259; p=0,132) и С-реактивного белка (ρ=0,133; p=0,598). В группе оИБС выявлена сильная связь уровней ФНО-α и VEGF (ρ=0,724; p<0,001) и слабая связь с уровнем общего ХС (ρ=0,105; p=0,467). Для определения возможной роли ФНО-α в прогнозировании ИБС был проведен ROC-анализ. Пороговое значение уровня ФНО-α в точке отсечения, которому соответствовало наивысшее значение индекса Юдена, составило 27,960 пг/мл. ИНОКА прогнозировалась при уровне ФНО-α выше данной величины. Чувствительность и спе­цифичность полученной прогностической модели составили 81,2 и 68,6% соответственно. В течение двухлетнего наблюдения частота развития неблагоприятных исходов была сопоставима у пациентов обеих групп. По результатам логистического регрессионного анализа, полученные модели не были статистически значимыми для всех наблюдаемых исходов при данном размере выборки и числе сердечно-сосудистых осложнений.

Заключение Согласно полученным данным, наиболее высокий уровень ФНО-α выявлен в группе пациентов с ИНОКА. По результатам ROC-анализа, ФНО-α является статистически значимым предиктором развития необструктивного поражения КА.
