<trans-abstract xml:lang="en"><p>Aim        To investigate the carrier status of the APOB rs693C&gt;T and PCSK9 rs562556G&gt;A single nucleotide variants (SNVs) and evaluate their association with lipid profile parameters and the incidence of cardiovascular events in patients (aged ≤44 years) with familial hypercholesterolemia (FH) and hyperlipoproteinemia(a) [hyperLP(a)].</p><p>Material and methods    We evaluated 166 relatives of index patients with premature cardiovascular disease (CVD). The cohort was stratified into three groups: patients with FH, patients with hyperLp(a), and a control group with normal lipid profiles. Participant ages ranged from 2 to 44 years, with pediatric patients comprising 50% of the sample. Clinical assessments, medical history evaluations, and serum lipid panel measurements, including apolipoprotein B (apoB) and lipoprotein(a) levels, were performed. Genotyping for the APOB rs693C&gt;T and PCSK9 rs562556G&gt;A SNVs was conducted using real-time polymerase chain reaction.</p><p>Results  Among patients aged ≤44 years with FH and hyperLp(a), a predominance of the PCSK9 rs562556 AA genotype, known to be associated with premature atherosclerosis, was observed. The AA+AG genotype of rs562556 was significantly associated with elevated low-density lipoprotein cholesterol (LDL-C) levels, as well as an increased incidence of CVDs in both the FH and hyperLp(a) cohorts. Furthermore, the prevalence of the APOB rs693 TT genotype was significantly higher in the FH group compared to both the hyperLp(a) cohort and healthy controls.</p><p>Conclusion         In patients aged ≤44 years with heterozygous FH and hyperLp(a), a predominance of the PCSK9 rs562556 AA+AG genotypes is significantly associated with elevated apoB and LDL-C levels, as well as the premature development of atherosclerotic CVDs. Patients with FH were characterized by the carriage of the APOB rs693 TT genotype.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>Семейная гиперхолестеринемия</kwd><kwd>гиперлипопротеинемия (а)</kwd><kwd>высокий риск развития сердечно-сосудистых заболеваний</kwd><kwd>дети</kwd><kwd>молодые взрослые</kwd><kwd>аполипопротеин В</kwd><kwd>пропротеинконвертаза субтилизин/кексин 9 го типа</kwd><kwd>первичная профилактика сердечно-сосудистых заболеваний</kwd></kwd-group><kwd-group xml:lang="en"><kwd>Familial hypercholesterolemia</kwd><kwd>hyperlipoproteinemia(a)</kwd><kwd>high cardiovascular disease risk</kwd><kwd>children</kwd><kwd>young adults</kwd><kwd>apolipoprotein B</kwd><kwd>proprotein convertase subtilisin/kexin type 9</kwd><kwd>primary cardiovascular prevention</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Финансирование Отсутствует.
