Проведен сравнительный анализ параметров внутрисердечной гемодинамики у пациентов с разной степенью ожирения, разной длительностью СД 2‑го типа, в возрастных группах пациентов моложе 60 и старше 60 лет.</p><p>Результаты   У всех наблюдаемых пациентов выявлена диастолическая дисфункция как ЛЖ, так и ПЖ. С возрастом и увеличением длительности СД 2‑го типа отмечено прогрессирование диастолической дисфункции, преимущественно по рестриктивному типу. Повышение ИМТ, наоборот, сопряжено с увеличением объемных показателей желудочков. Отмечено, что именно модифицируемые факторы риска (ожирение и СД 2‑го типа), но не возраст, в большей степени способствуют нарушению релаксации ПЖ.</p><p>Заключение     Стратегия нормализации массы тела и параметров углеводного обмена является приоритетной в борьбе с возникновением и прогрессированием хронической сердечной недостаточности у больных из групп высокого риска.</p></abstract><trans-abstract xml:lang="en"><p>Aim      To study early manifestations of left ventricular (LV) and right ventricular (RV) myocardial remodeling in high-risk patients.</p><p>Material and methods  Intracardiac hemodynamics was studied by equilibrium radionuclide ventriculography (ERVG) in 83 patients (mean age, 61.1±8.9 years) with preserved LV ejection fraction according to echocardiography data, a body weight index (BWI) &gt;25 kg /m2, obesity, and type 2 diabetes mellitus (DM2). Parameters of intracardiac hemodynamics were compared in patients with different degrees of obesity and DM2 durations in age groups of younger and older than 60 years.</p><p>Results All patients had both LV and RV diastolic dysfunction. The diastolic dysfunction progressed with age and DM2 duration, primarily by the restrictive type. The increase in BWI, in contrast, was associated with increases in ventricular volumetric parameters. It was noted that specifically modifiable risk factors (obesity and DM2), but not the age, mostly facilitated the impairment of RV relaxation.</p><p>Conclusion      The strategy of normalizing the body weight and carbohydrate metabolism is priority in combatting the development and progression of chronic heart failure in high-risk group patients.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>Ожирение</kwd><kwd>индекс массы тела</kwd><kwd>возраст</kwd><kwd>сахарный диабет 2-го типа</kwd><kwd>радионуклидная равновесная вентрикулография</kwd></kwd-group><kwd-group xml:lang="en"><kwd>Obesity</kwd><kwd>body weight index</kwd><kwd>age</kwd><kwd>type 2 diabetes mellitus</kwd><kwd>equilibrium radionuclide ventriculography</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Источники финансирования отсутствуют.
