При этом доказательства эффективности лечения ДЖ у стабильных пациентов с ХСН получены только для внутривенно вводимого железа карбоксимальтозата, что в том числе указано в рекомендациях по лечению ХСН 2020 года, одобренных Научно-практическим Советом Минздрава РФ. Публикация результатов исследования AFFIRM-AHF, которые подтвердили целесообразность тактики начала устранения ДЖ непосредственно после эпизода декомпенсации ХСН с целью снижения риска последующих госпитализаций по поводу утяжеления ХСН, стала основанием для написания данного документа. Эксперты пришли к заключению что применение внутривенно железа карбоксимальтозата у пациентов с сочетанием ХСН с ФВЛЖ &lt;50% и ДЖ, на фоне декомпенсации ХСН, позволяет предотвращать последующие декомпенсации. У этой категории пациентов целесообразен скрининг ДЖ и, при его выявлении, назначение железа карбоксимальтозата. У стабильных пациентов с ХСН, имеющих ДЖ, внутривенное применение железа карбоксимальтозата улучшает качество жизни и уменьшает клинические проявления ХСН, в связи с чем скрининг ДЖ и применение данного препарата целесообразно рассмотреть у таких пациентов.</p><p> </p></abstract><trans-abstract xml:lang="en"><p>In recent years there has been significant interest in treating iron deficiency (ID) in patients with heart failure (HF) due to its high prevalence and detrimental effects in this population. As stated in the 2020 Russain HF guidelines, Intravenous ferric carboxymaltose remains the only proven therapy for ID.This document was prompted by the results from the recent AFFIRM-AHF trial which demonstrates that treatment of ID after acute HF decompensation reduces the risk of future decompensations. Experts have concluded that in HF patients with acute decompensation, a left ventricular ejection fraction of &lt; 50% and ID, Intravenous ferric carboxymaltose reduces future HF hospitalisations.
